Botulinum toxin is produced by Clostridium botulinum bacteria. There are four commercial preparations of botulinum toxin type A which are commonly used. Botox and Vistabel manufactured by Allergan Inc. Botox has become the most investigated toxin on the market today, with excellent safety profiles. Dysport is also available as is Azzalure which is made in the UK.

How does it work?

The toxin acts on the junction between the nerves and muscles preventing the release of one of the chemical messengers called acetylcholine from the nerve endings which would normally cause the muscle to contract.

If the messenger is prevented from being released this results in weakened muscle activity.

When injected in very small quantities into localised areas of the face botulinum toxin Type A will reduce wrinkles and furrows in the skin by weakening the muscle activity responsible for them. It should not obliterate facial expression or stop muscles from functioning but modify expression so that wrinkles are less severe.

Cosmetically the aim is to leave facial expression intact but soften the appearance of wrinkes around the forehead, eyes and mouth predominantly.

Botulinum toxin type A is most beneficial in the upper third of the face where it is extremely effective in reducing wrinkles and furrows in the region of the brow, root of the nose and Crow’s feet area.

It can also be helpful in preventing drooping at the corners of the mouth and reducing the visibility of transverse wrinkles in the upper lip (“smokers lines”). It can also be used in the neck with impressive rejuvenation!

In addition to treating facial lines and wrinkles, botulinum toxin is very helpful in preventing excess sweating (technically known as hyperhydrosis) and in treating migraine headaches.

The administration of botulinum toxin Type A is via a series of small injections which cause little discomfort and take little time. We have a lovely calming environment and lots of tricks to make this as comfortable as possible for you.

For most individuals the results take 24-48 hours before a reduction in muscle activity is noted but in some it may take as long as a week before it begins to work.

The duration of action for most is between 3-6 months and varies between individuals. It is helpful to exercise the treated muscles as much as possible for the first hour or two after the injection and, when it is used in the face, not to lie down for 3-4 hours after the treatment. We will advise you on the do’s and don’ts while you are with us.

Complications are extremely rare. Allergy is reported but exceptionally rare. The most talked about complication is of weakening a muscle unintentionally, such as the one which opens the upper eyelid, creating a drooping eyelid as a result. Careful technique should avoid this but if it were to occur, it is treatable with eye drops and resolves after approximately 2 weeks.

Dermal fillers have become increasingly popular to reduce the visibility of facial lines and wrinkles since the introduction of “Collagen” in the late 1970’s. Hyaluronic acid based fillers are now the most widely used and least problematic in clinical practice.

Hyaluronic acid (HA) is a naturally occurring substance found in many parts of the body but it is manufactured synthetically for use as a dermal filler. Many products are available but I only use non permanent fillers in the practice which are approved by the FDA. I encourage a subtle, natural look.

HA fillers are injected into or beneath the skin.  Some fillers are used to fill fine lines and wrinkles in the face and hands and to add volume to the cheeks, chin and lips.

After injection there may be a little redness and, occasionally, bruising but this will usually resolve in 24 hours or so. It is impossible to eradicate wrinkles with fillers but they can be made significantly shallower. Micro filling can be very effective and subtle in lips if you desire a slightly fuller lip with very subtle results…

HA fillers will last for 3-6 months in most people.  Complications are extremely rare though infection is known. There may be a little unevenness at the injection site for a short time. Allergy is virtually unknown and “test” injections are unnecessary.
